Conclusion
For the duration of a file being open, an Action Plan is developed and continuously revisited; actions and milestones are set with the young person, caregiver and involved supports.
Coordinated Custom Case Conferences (C4) occur every 4 – 6 weeks to facilitate progress and respond to changing needs as necessary, and to-do/action assignments are given to all participants.
TRiP will remain involved with a young person until there is a point of mutually agreed upon stability, at which time there is a transition plan put in place and TRiP will move to close the file.
